Dragon Age; Origins is as dark as the original Baldur's Gate games, but 3D. Very bloody. You can murder or threaten just about anyone who has voice acting, even would-be-companions.
I think there are only two companions who you cannot kill, but they can both abandon you depending on choices you make
Your origin also gives you unique perspective into the world, as well as opening up certain paths and conversations with characters you encounter.
Probably one of the last good western RPGs
