Such cases are quite common, games are stupidly complex things. Think like car writing, but instead of wiring a single car, you are networking a whole fleet of cars onto each other, and there are cars inside cars inside cars.

But nobody in the project really understands all of the complexity. Individual mechanics just did their simple jobs and it piled/tangled up, became sentient, etc...
